Introduction: Ischemia-modified albumin (IMA), a new marker of myocardial ischemia, is known to elevate during ischemia induced by percutaneous coronary intervention. It is, however, not known whether ischemia-modified albumin also elevates during transient coronary vasospasm. IMA is highly sensitive for the identification of myocardial ischaemia in patients presenting with symptoms of acute chest pain. IMA is also a good alternative to D-dimer in the exclusion of PE identification in terms of cost and efficiency. When used in combination with clinical probability scores, it has a similar positive effect on sensitivity to that of D-dimer and a superior one to that of NPV.

Principle of the Assay: The microtiter plate provided in this kit has been pre-coated with an antibody specific to IMA. Standards or samples are then added to the appropriate microtiter plate wells with a biotin-conjugated polyclonal antibody preparation specific for IMA and Avidin conjugated to Horseradish Peroxidase (HRP) is added to each microplate well and incubated. Then a TMB (3,3'5, 5' tetramethyl-benzidine) substrate solution is added to each well. Only those wells that contain IMA, biotin-conjugated antibody and enzyme-conjugated Avidin will exhibit a change in color. The enzyme-substrate reaction is terminated by the addition of a sulphuric acid solution and the color change is measured spectrophotometrically at a wavelength of 450 nm +/- 2 nm. The concentration of IMA in the samples is then determined by comparing the O.D. of the samples to the standard curve..
