Introduction: Kim-1 (kidney injury molecule-1) is a type I transmembrane glycoprotein expressed on dedifferentiated renal proximal tubule epithelial cells undergoing regeneration after toxic or ischemic injury. The extracellular domain of Kim-1 is composed of an immunoglobulin-like domain topping a long mucin-like domain, a structure that points to a possible role in cell adhesion by homology to several known adhesion proteins. Two splice variants (a and b) of the human Kim-1 having identical extracellular domains, differ in their cytoplasmic domains and tissue distributions. In normal kidneys, Kim-1 is undetectable, but this molecule is abundantly expressed in PTC after ischemic and nephrotoxic injury. In mice, Kim-1 is upregulated in polycystic kidney disease especially in regions of the kidney where there is fibrosis. In humans and in rodents, Kim-1 is induced after ischemic or toxic acute tubular necrosis and the cleaved Kim-1 ectodomain is quantified in the urine as a biomarker for acute kidney injury. The early and abundant tubular expression following different types of injury makes Kim-1 a specific marker of tubular cell injury that may be linked to recuperative or damaging mechanisms that direct the process of interstitial damage.

Principle of the Assay: The microtiter plate provided in this kit has been pre-coated with an antibody specific to Kim-1. Standards or samples are then added to the appropriate microtiter plate wells with a biotin-conjugated antibody preparation specific for Kim-1 and Avidin conjugated to Horseradish Peroxidase (HRP) is added to each microplate well and incubated. Then a TMB (3,3',5,5' tetramethyl-benzidine) substrate solution is added to each well. Only those wells that contain Kim-1, biotin-conjugated antibody and enzyme-conjugated Avidin will exhibit a change in color. The enzyme-substrate reaction is terminated by the addition of a sulphuric acid solution and the color change is measured spectrophotometrically at a wavelength of 450 nm +/- 2 nm. The concentration of Kim-1 in the samples is then determined by comparing the O.D. of the samples to the standard curve.
